Here is the main thread. The 4th attachment there tells you what to look for. Bad part numbers end with AC or AD. The CSP ended this past January.

Example bad one:

Took truck into the dealer. LVB tested bad and was replaced. thanks for the advice. Makes me wonder if I'll have to replace it every 2yrs/20k miles.
