One in a million failure, tear it open look for fractured contact or bar running between the wire lugg and the ground spring clip assembly. The current should be near nil on the ground when there are no shorting conditions.

Yeah I think I’m going to send it to Martin Meek who I do our podcast with he wanted to tear it down and check it out. He does Martin Rebuilds and is an engineer so it will be interesting to let him take a look

It seems like I've seen more reports of Hubbell going than everyone else's receptacles but I've also read that Hubbells are often counterfeited so one step is to make sure you get them from a reputable retailer.
I've never needed to disassemble any other receptacle for routine maintenance but then, the EV outlet is likely the highest continuous demand receptacle at home.
