It does charge when it gets below a certain voltage for a certain amount of time. I’ve seen it occur a few times on mine. Lots of threads early on about this process, especially how it waits too long before it starts to charge.
I never had issues so I can't say if the BMS CSP did anything but I thought it was supposed to address low voltage on the 12V battery.

Also, I think there have been other updates to the way LVB charging is handled. For instance, I believe Accessory Mode used to be on 12V but it is now on full system power? I could be mistaken...

That could be a "setting's" issue. Could that be adjusted in/with Forscan?

Maybe, but I need the ICE guys to figure it out!

Agree, I suspect we have received OTA's that have improved this. I recall someone mentioning they saw the DC-DC charger operate in Accessory mode. And it does appear that the inhibit updates might be engaging the DC-DC charger now whereas before we frequently got failed updates for 12V too low.

I've always thought it is strange that the charge in the 12V battery isn't maintained (trickle charged when needed) when the vehicle is merely parked. Does anybody know why this is?

Twice in the winter my truck has died because the 12V went dead when parked in really cold weather for a couple days. It feels so STRANGE to have to use a battery booster to get the truck running! LOL! Then you drive it for a bit and then its just fine.
Ford needs to adopt what Genesis-Hyundai does. They have an indicator light on the top of the dashboard that lets you know that the 12 volt battery is charging when needed when the vehicle is turned off!

Really a shame. I've had 3 vehicles where the 12v battery ran down where eather a charge cable was hooked up and/or a huge onboard HVB was available. There is no excuse for this unless the HVB is dead or low as well. Come on KIA, Nisson, and Ford, you are all better than this. Every time a 12v lead acid goes dead, it looses likely half its life.

Ford needs to update their software so the hvb can turn on and charge the 12v battery. My Genesis does that. A small indicator near the center speaker on dash will blink when charging the 12v battery.
