The buyer of the truck, you.

Just list your name twice, doesn't affect anything as far as I recall, they might have this in the case of business owner, where the president or other authorized officer is empowered to make applications and contract on behalf of the entity.

Yes, I just looked at my application and its:

Name of Signatory: SAME AS ABOVE
Title of Signatory: Mr
Signature of Vehicle Purchaser/Lessee: MY SIGNATURE
Email of Vehicle Purchaser/Lessee: MY EMAIL
Date: DATE OF PURCHASE
