Some update along the line, erased the door access code.
This happened to me also, details here: https://www.f150lightningforum.com/...-your-backup-start-passcode.31723/post-582850

Ignore the people trying to gaslight you into thinking it was something you did or did not do. There absolutely was an update that erased entry codes and backup start passcodes at some point. I do not know which one, and @Ford Motor Company never got back to me with an answer about how this happened, and what the engineers are doing to make sure it does not happen again.

There is a video from McArthur Long Ford about how to get your master code, as long as you have two different fobs.

No update erased factory door codes. Yes, updates have erased customer-set codes.

Please update your Account Details to include the year of your truck. Seems like in 2024 they stopped printing cards with the keycode. But there is a thread here that shows how to recover the master code without going to the dealer.
You can also use the brute force method and actually read the code on the side of the BCM:

Unless your BCM has been replaced, this code will always work as it is uneditable firmware in the BCM. You cannot erase this one with an update.

Very good to know if you didn't buy your truck new and/or don't have the card.

I went to get some blood work done early in the morning. Used the PAK to get there. Standing outside, waiting for the office to open, within eyesight of the truck. I noticed that the lights blinked and the horn honked on the truck. I asked myself, why would that happen?
I finished the blood work. On the way out to the truck I realized I didn't have my phone. I had left my phone in the truck.
I tried the door entry code I had set a long time ago. It didn't work!! None of any of the possible codes I might use worked!!
So.. the conclusion of all this is...

Some update along the line, erased the door access code. Unless you need the door code you don't know that it has been erased. Check it periodically. I didn't get a printed manual so there is a no place to look for the required "factory code" that is needed to set the door access code. The dealer here, wanted to charge me an hours labor for this code. I had to convince them that an update erased it, therefore I should not have to pay for the code.

The phone is an android, and it seems that when the phone goes/went to sleep, it disconnects from the truck. Hence the reason the truck locked itself. The ford app, it seems, is unable to be active in the background.

Had anyone else had this happen while leaving the phone in the truck?

What is the proper behavior when a phone is left behind?
Should it stay open until the phones dies?
Should it have time limit?
Should it behave as it did, when the phone sleeps, the truck locks?
Should it behave as it did, if and only if there is a non "factory" door code?
Do you wear an Apple, Samsung or an Android (not sure if ALL Android OS watches have access) watch? The Ford app can also be locked or unlocked from the watch too.

I had no idea about this, so thank you for that!

You mean the vehicle can be locked or unlocked from the watch.

Ok, that's pretty damn cool. I had never tried using the Ford App on my watch, so when I tried to use it the first time, it prompted me to open the app on my phone. I did that, a second later the app updated. For those that haven't done it yet, you get 3 different screens: 1 showing SOC/GOM, 1 showing lock/unlock/climate, 1 showing location of vehicle.

Note that the Apple watch app and the Android watch app are totally different. The Android app's 3rd screen shows location, lets you hit the panic button, and open GoogleMaps.

Apple watch does the same. I just gave a simplified answer.
